The short answer: Low iron often shows up as tiredness, breathlessness, headaches, restless legs or hair shedding. These signs are common and have many causes. The only way to know is a ferritin blood test, read against the 2026 cut-off of 30 ng/mL.
Why are people searching for low iron symptoms now?
The "ferritin face" trend made millions of people stare at their dark circles. Doctors were quick to say your face cannot diagnose low iron.
On 28 October 2026, Women's Health UK asked a GP what the real signs are [1]. Dr Stephanie Roche of HCA Healthcare UK said your reflection cannot tell you if your iron stores are low [1]. She listed the symptoms that matter more.
We looked at the face claims in our post on ferritin face. This post is the follow-up. It covers the signs worth noticing, and the test that settles it.
What are the most common symptoms of low iron in women?
Iron helps make haemoglobin, the protein in red blood cells that carries oxygen. When iron runs low, your body gets less oxygen where it needs it.
The American Society of Hematology (ASH) lists these common symptoms [2]:
- Lack of energy.
- Shortness of breath.
- A fast heartbeat.
- Headaches.
- Pale skin.
The Women's Health UK piece added a few more that women often brush off [1]. Think persistent tiredness, getting tired faster during exercise, dizziness and trouble concentrating.
Picture this. You used to climb three floors to your flat without thinking. Now you stop on the second landing to catch your breath. That change is worth noting.
Which low iron signs do women often miss?
Some signs do not sound like iron at all. Women's Health UK listed these lesser-known ones [1]:
- Restless legs, an urge to move your legs at night that makes it hard to fall asleep.
- Hair thinning or more hair in the shower drain.
- Brittle nails.
- A sore tongue or cracks at the corners of your mouth.
- Odd cravings for ice, chalk or paper, called pica.
Restless legs has some research behind it. In a 2022 French study of 554 women who had periods, ferritin of 50 or lower was linked to restless legs [3]. Those women had nearly three times the odds of reporting it [3].
The same study linked recent hair loss to ferritin of 20 or lower [3]. We cover that in our post on low ferritin and hair loss. If you crave ice or mitti, read our post on iron deficiency cravings.
Can you have low iron symptoms with normal haemoglobin?
Yes. This is the part many people miss. Your iron stores can run low for months before your haemoglobin drops.
In that French study, none of the 554 women were anaemic [3]. Yet 54.9% had ferritin below 50 [3]. So more than half had low-ish stores with a "normal" blood count.
And it can still make you feel worse. A 2012 trial in France studied 198 tired women with ferritin below 50 and normal haemoglobin [4]. Half took 80 mg of iron a day for 12 weeks. The rest took a dummy pill [4].
Tiredness scores fell by 47.7% with iron and by 28.8% with the dummy pill [4]. So iron helped more, but the dummy pill helped too. Iron did not change mood, anxiety or quality of life scores [4].
Our post on normal haemoglobin but low ferritin goes deeper on this.
Why can't a symptom checklist tell you if your iron is low?
Because every sign on the list has other causes. Tiredness can come from poor sleep, a thyroid problem, low vitamin B12, stress or a new baby. Headaches and dizziness have dozens of causes.
A checklist is useful for one thing. It tells you when to ask for a test. It cannot give you the answer.
That is also why we would not start iron tablets on a hunch. If your tiredness comes from your thyroid, iron will not help it.
Which blood tests should you ask for?
Ask for a complete blood count (CBC) and a serum ferritin test. Ferritin is the protein that stores iron, so it shows how full your iron savings account is.
In September 2026, ASH set new cut-offs [5]. A ferritin of 30 ng/mL or lower now means iron deficiency in adults, including women who have periods [2]. A cut-off of 50 ng/mL applies to high-risk groups, such as women with heavy periods [2].
Ferritin can look falsely high when you have an infection or inflammation. In that case, ASH suggests also checking transferrin saturation, a measure of how much iron is moving in your blood [2]. Iron deficiency is likely if ferritin is under 100 or transferrin saturation is under 20% [2].
How common is low iron in Indian women?
Very common. The National Family Health Survey 5 (2019 to 2021) found that 57% of Indian women aged 15 to 49 were anaemic [6]. That is more than one in two.
Anaemia is the late stage, when haemoglobin has already dropped. Many more women likely have low stores before that point. So if you have symptoms, a test is a sensible step, not an overreaction.
What this means if you...
...tick three or four boxes on this list
Book a CBC and a ferritin test. Take the report to your doctor and ask how it compares with the 2026 cut-off of 30 [2]. If iron is fine, ask what else to check, such as thyroid and B12.
...were told your report is "normal"
Look at the actual ferritin number. Many lab ranges in India still start at a lower value. A ferritin of 20 can be marked normal and still be below the new ASH cut-off [2].
...are trying to get pregnant
Ask for your ferritin to be checked. Women's Health UK notes that low ferritin has been linked to fertility trouble and pregnancy complications [1]. Your doctor can advise on what level to aim for.
Where AIM stands
We are launching FORGE soon. It is one peach-mango sachet a day with 9 mg of iron as iron bisglycinate. It also has 80 mg of vitamin C, 5 g of creatine monohydrate and BioPerine. Nine milligrams is a daily top-up, not a dose for fixing a diagnosed deficiency. In a 12-week trial of 480 women, 18 mg of iron bisglycinate raised ferritin less than 60 mg of ferrous sulfate [7]. A gentler form is not a shortcut around the right dose. If these symptoms sound familiar, start with a ferritin test and your doctor. You can see where FORGE may fit later on our find your routine page.
Frequently asked questions
What are the first signs of low iron in women?
Early signs are often vague. They include tiredness that rest does not fix, getting breathless on stairs, headaches, dizziness and trouble concentrating. Some women notice restless legs, more hair shedding or brittle nails. These signs have many other causes, so a ferritin blood test is the only way to confirm low iron.
Can you have iron deficiency without anaemia?
Yes. Iron stores can run low for months while haemoglobin stays normal. In a French study of 554 non-anaemic women with periods, more than half had ferritin below 50. A ferritin test catches this earlier stage. A haemoglobin test alone will miss it, so ask for both.
Is restless legs a sign of low iron?
It can be. A 2022 French study found that women with ferritin of 50 or lower had nearly three times the odds of reporting restless legs. Restless legs means an urge to move your legs, often at night. Other causes exist too, so mention it to your doctor and ask for a ferritin test.
What ferritin level is considered low for women?
The 2026 American Society of Hematology guideline says ferritin of 30 ng/mL or lower means iron deficiency in adults, including women who have periods. A cut-off of 50 applies to high-risk groups such as women with heavy periods. Your lab report may use an older range, so ask your doctor to compare.
Should I take iron if I have these symptoms?
Test first. Tiredness, headaches and breathlessness can come from thyroid problems, low B12, poor sleep or stress, and iron will not help those. A CBC and ferritin test show whether low iron is the cause. If it is, your doctor can set the right dose and recheck your levels.
